<h4>How you'll change lives</h4>As a Registered Nurse (RN) at US Renal Care, you'll be an integral part of a cross-functional team, working under the supervision of a Charge Nurse and assisting and supervising Patient Care Technicians (PCTs) to care for patients living with kidney disease.<h4>What you'll be doing</h4>Patient care. You'll work directly with patients to provide safe, comfortable, and high-quality dialysis treatment. Responsibilities include completing and documenting physical assessments before and after treatments and monitoring patients during treatment. You'll ensure quality renal care by completing monthly reviews of patient medication profiles and administering medications according to physician orders. Teamwork. You'll be a critical member of an interdisciplinary team to provide all required patient assessment and care planning activities. You'll also make sure center staffing requirements are met at all times under the direction of the Administrator and according to state regulations for staffing ratios.Safety &amp; Quality. Working under the supervision of a Charge Nurse, you'll adhere to all company policies and procedures and state and federal laws and regulations. You'll participate in all required staff meetings and continuing education offerings.

Skills include demonstrating a comprehensive understanding of the department’s immediate needs and long term growth goals, showing initiative, using critical thinking to problem solve independently, exhibiting leadership qualities, and effectively engaging with the clinical team.Essential Duties and Responsibilities:<ul><li>Responsible for the general oversight, assessment, planning, and construction of the nursing care delivered to the participant.</li><li>Assess a patient through obtaining history, performing appropriate tests, making or collecting observations from the participant and caregivers, and consulting with other members of the IDT.</li><li>Teaches patients and families health care regimens. Explains and provides information regarding disease and wellness, tests, results, medications, diet, exercise, and other aspects of the plan of care to patients.</li><li>Safely and effectively administers medications, tests and treatments as ordered. Manages the refills of all patient medications with appropriate authorization by the provider.</li><li>Provide and coordinate health teaching required by individuals, families and groups to maintain the optimal possible level of health.</li><li>Completes biannual and status change nursing assessments for an assigned panel of participants in accordance with Care Planning Policy and documents problems, goals &amp; interventions in the participant’s care plan.</li><li>Utilizes Assessment tools including but not limited to PACE Nurse Assessment and Medication Management to collect data.</li></ul>Qualifications and Requirements:<ul><li>Baccalaureate degree in nursing preferred. Current Massachusetts license in nursing.</li><li>Current BLS certification required.</li><li>Minimum of 2 years experience in ambulatory, home health, hospital or sub-acute setting preferred, equivalent training or related experience considered.</li><li>A minimum of one year experience working with a frail or elderly population.</li><li>Prior experience in interdisciplinary team setting a plus.</li></ul> NeighborHealth opens our doors to people from all demographics and cultures.
